Specify subject(s), goals (e.g. grade improvement, test score target), and session format.
Scheduling and cancellation policy
Require minimum cancellation notice and define a late-cancellation or no-show fee.
Session location/platform
State whether sessions are in-person, at a library, or via video call, and what's needed (whiteboard app, shared documents).
Progress reporting
Note how/whether you'll update the parent on progress (e.g. brief notes after each session).
Materials
Clarify who provides textbooks, practice tests, or worksheets.
Payment terms
Specify rate, billing cadence (per session vs package), and accepted payment methods.
