Personal Training contract template

A short client agreement sets expectations on packages, cancellations, and physical risk before the first session, which matters more here than most solo services since you're directing someone's physical activity.

Session package and pricing

Spell out the price per single session vs. any prepaid package (e.g., 10-pack), how long package sessions stay valid, and whether package sessions expire.

Cancellation and late-arrival policy

Require a minimum cancellation notice (typically 24 hours) with a late-cancel or no-show fee, since an empty session slot can't be resold on short notice.

Health history and liability waiver

Have every new client complete a PAR-Q (Physical Activity Readiness Questionnaire) or similar health history form, and sign an assumption-of-risk / liability waiver before the first session.

Scope of services

Clarify that you provide fitness coaching, not medical, dietetic, or physical therapy advice, and that clients should consult a physician for medical conditions or injuries.

Location and equipment

State where sessions take place (your facility, client's home, a public gym, outdoors) and who provides equipment, since this affects both liability and your pricing.

Payment terms

Specify payment due dates for packages, accepted payment methods, and your policy on refunds for unused sessions.

This is general guidance, not legal advice. Consider having a local attorney review your final agreement.
