A short intake and service agreement protects both sides of a hands-on service — it sets expectations on health disclosures, cancellation, and scope before the client is on the table.

Scope of service

Describe the modality(ies) being offered (Swedish, deep tissue, sports, prenatal, etc.) and state plainly that services are for relaxation and therapeutic bodywork, not a substitute for medical diagnosis or treatment.

Health intake and contraindications

Require a health history/intake form before the first session, covering conditions (pregnancy, recent surgery, blood clots, skin conditions) that change or rule out certain techniques.

Session length and pricing

State the session length booked, the price for that length, and any add-on fee for mobile/in-home travel.

Cancellation and no-show policy

Specify the notice window required to cancel or reschedule without a fee (24 hours is standard) and the no-show/late-cancellation charge, since a missed appointment is unrecoverable income for a solo practitioner.

Draping and consent

State your draping practices clearly and that the client can request a technique change, pressure adjustment, or end the session at any time — this protects both parties and is standard professional practice.

Payment terms

Specify accepted payment methods and when payment is due (at time of service is standard), plus your policy on package/prepaid session expiration if you sell multi-session packages.
